I always start with correcting the basic grammatical mistakes in spoken English by taking up easy topics from daily life for discussion or by initiating a general conversation where I and the student will talk just about anything for long and I will keep informing them about the errors and also give them solutions as per their choice and convenience so that find the corrections compatible to their speaking styles and then later I move on to the accent correction and start by rectifying the pronunciation defects in commonly used words by asking them to speak about anything for a specific time frame, keep collecting data and then conduct one on one sessions with each student as accent related issues are different in each case. Later when I find them good with their voice & accent and expressions then I start small group discussions so that they grow in confidence when it comes to speaking with other people in day to day life.
